Medical
Full time salaried and hourly employees are eligible for medical coverage the first of the month after hire. The medical plan is a “PPO” or Preferred Provider Plan. Use of in-network medical providers results in lower costs for you. Includes office visits, prescription drug coverage and urgent and emergency room care as well as inpatient hospital coverage. Lifetime Maximum of $1 million. Istonish contributes up to 78% towards employee medical coverage rates.
Dental
Full time salaried and hourly employees are eligible for dental coverage the first of the month after hire. Two plans are offered, a DMO (Dental Maintenance Organization) and a “PPO” Indemnity Plan.
Vision Care
A voluntary vision plan is offered. Benefits include routine vision exams and preferred pricing on a large selection of brand-name, designer frames, lenses and lens options.
Disability Insurance
Istonish provides full-time salaried employees with short and long-term disability income benefits, and pays the full cost of this coverage. In the event that you become disabled from a non work-related injury or sickness, disability income benefits are provided as a source of income.
401(k)
To help you prepare for the future, Istonish sponsors a 401(k) plan. As a full or part-time employee, you may start participating in this plan on the first day of the calendar quarter and after completing six months of service. You are 100% vested immediately, and in addition, Istonish will match your salary deferral dollar for dollar up to 4% of your salary.
Life Insurance
Istonish provides full-time salaried and full-time Istonish, Inc. hourly employees with group life and accidental death and dismemberment (AD&D) insurance, and pays the full cost of this benefit.
